The old dispute between MS/MD and FCPS has returned with new induction. The competition for a limited number of postgraduation seats grows more intense with each passing season. Most of the candidates have passed both FCPS Part 1 and JCAT. Those who have gotten into BHU/RHC through some miraculous means pick FCPS slots, while the rest of them are stuck with the MS/MD option. If you are the unlucky fellow who fails to grab BHU/RHC and does not have connections at some private hospital, you are left with one option and everyone around you will try to stop you from taking it. FCPS from a private institution is often recommended above MD/MS at public hospitals. If FCPS from a private hospital is superior to MD/MS from the government, why is MBBS from a private college considered inferior to that of the public? There is no difference between the two courses (FCPS,/MD/MS) in terms of roster, ward rounds and training time. Training in both programs is equally unstructured and random. Surgery and allied specialities are particularly prone to favouritism.
Both have intermediate and exit exams. FCPS exams are more organized, and their passing rate is high. What really put MS/MD down is research work. Nobody will ever teach or guide you on how to conduct research; instead, they will just advise you to avoid choosing MD/MS. Synopsis topic selection is a tough nut to crack and the university-required study design is notoriously difficult to follow (Randomized Control trial, comparative study, experimental study, Quasi-Experimental study). No observational studies are accepted, and the study must have not been done in any hospital under UHS in the previous 5 years. How can a candidate choose a fresh topic when methods have not evolved, and the procedures being performed in our hospital have not changed in decades? Only three procedures your professor do laparoscopically have already been compared with open at some other hospital. The angles your consultant use to put the ports and make dissection in Calot’s triangle have already been occupied.
Moreover, due to candidates' lack of comprehension of research work and supervisors' lack of interest, the experience is generally negative and frustrating. Even if you succeed and pass your exam, your future is still in doubt. Compared to FCPS, opportunities are limited, and your own coworkers will view you as less capable. Clinical research is supposed to help you improve as a doctor and advance your career, but in our country, the opposite is true. I do not know about GULF countries, but you are not going to acknowledge by your own colleague and ward any soon, no matter how skillful you are!
